I,_________________________, having entered the Serenity House or House of Freedom and Miracles program in Cincinnati, Ohio, on ___/___/___, agree to follow through with this Continued Care Agreement upon  my successful completion of the in-house phase.   I recognize that the assistance and help given to me by the many referral sources while living at Serenity House  or House of Freedom and Miracles was only the beginning of the solution and not the end of the problem.  Given this, I know that I must receive continued care in order to increase my success chances in a life-long recovery program of my choice.

        I agree to attend the Sunday night continued care meeting from 7:30 PM until 9:30 PM for eight consecutive weeks and afterwards to participate in the Recovery Dinner once a month for four consecutive months.  This will allow me six months of continued support in my recovery;

        If living in the Hamilton County area upon successful completion of this program, I will continue meeting with staff on Fridays to discuss 12-step, sponsor, and counseling meeting attendance.  I will make plans in advance of what time this session will occur.  

       Every resident that leaves the Serenity House or House of Freedom and Miracles, regardless of type discharge, does less for their recovery than they were doing prior to discharge. That is 100%.  Both NA & AA suggest 90 meetings in 90 days.  The day you leave this program is a perfect day to begin your 90 in 90.

For those who choose not to remain in the Hamilton County area an alternative continued care plan will be agreed upon.  All residents who leave the house successfully or administratively will complete a discharge plan. The plan development will begin at least two weeks prior to discharge.
